Bilateral TECNIS MF versus Customized-TECNIS MF and ReZoom; Achieving Spectacle Independence

Purpose: To study Clinical Outcomes- Patient Satisfaction- Accuracy-Efficacy-Safety, after Refractive Lensectomy with the TECNIS Multifocal lens: a foldable diffractive Iol with a modified prolate anterior and effective reading add of 3, 2 D. and the Rezoom lens; a refractive lens with an effective reading add of 2,8 D.
Methods: 106 of 53 patients (sf.eq. +4,21D. ± 1,3 D) implanted simultaneously and bilaterally with a TECNIS Iol (group I)and 52 eyes (sf.eq.+3,25 ± 1,2 D) of 26 patients (group II) with a customized approach TECNIS- ReZoom with a 1 – 2 weeks interval were analyzed at 3 months. Distance uncorrected and best corrected, Near Ucva and Bcva as well as reading rate and reading speed were analyzed and compared. Evaluation of secondary effects and satisfaction degree was studied.
Results: Ucva improved from 20/80 preop. to 20/24 in group I and from 20/85 to 20/22 in group II. Distance corrected, 103/106 eyes could read(monocular) J1 in group 1 and 36/52 eyes in group 2. At intermediate distance 49/53 patients in group 1 and 26/26 patients in group 2 could read J3. Important side effects were exceptionally present: 8% in group I and in 6% of group II. The satisfaction degree was either excellent (90%) or good (10%) in both groups. In group 1 – 49/53 patients were spectacle free; in group II 26/26 patients. No eye lost vision.
Conclusion: TECNIS ZM is safe and efficient for Refractive Lensectomy in Hyperopia and Presbyopia. Customizing by matching and mixing refractive -Rezoom -and diffractive -TECNIS MF- IOL's may enhance quality of vision and spectacle independence. The approach has to be tailored based on clinical measurements, patient's occupation and hobbies.
